 Is it >240/308or>240/352 to get 99

All those who r getting 99 r scoring >240. I want to know upper limit 308(if they randomly remove a block, is it true?) or 352 if all blocks counts.

They don't randomly remove blocks. There are experimental questions put in the exam randomly spread across all the blogs. There can be as much as 10-15% of experimental questions. Those questions are often not scored for the test over all.

Nobody really knows exactly how many questions get scored. The three digit score is not the number of questions you got right, but a score based on the bells curve set by the people that did the same exam you did.

there can be upto 20% experimental questions which are only experimental because they want to judge the difficulty level from the new pool of questions they are adding to the usmle every year. those questions will not be experimental once they judge the level of difficulty based on who has answered them correctly. so 80% of 352 is 282. the max score lies between 280-290s based on how many experimental questions were on your exam. therefore the 3 digit score could very well be the number you answered correctly but USMLE will not verify that.
