In the ER, preferred method of securing the airway is -- Orotracheal intubation.


In the field, often by paramedics - nasotracheal intubation.

however, for the OP question, it states nasotracheal has failed.. so orotracheal intubation is the way to go.

its not a good idea of learn it in order of succession as each are best (hence best answer) for different scenarios and learning those would help more.

In order to do "nasotracheal intubation" the pt. need to be breathing spontaneously. In apenic patient, it is CONTRAINDICATED.

extensive facial injuries, gurgling sounds - cricothyroidomy (surgical)

cervical spine injury - flexible bronchoscopy. This takes time so look at how much the pt. is in respiratory distress. If apneic or really severe distress.. orotracheal intubation can be still done with manual cervical stabilization.

neck injury - subcutaneous emphysema - flexible bronchosopy
