I think $4000 is excessive. My total cost is about $2000, including premium processing, $1000 is attorneys fees. Fish around, you may find a better deal

My hospital paid for the H1, I paid only for the premium processing.

First my hospital said I should pay . I started making some phone calls and telling them there's no way I can find 4000 $ so late in the game and in the end they said they would pay . I paid for the Premium Processing though. The processing at USCIS took exactly 2 days . The H1B and my husband's H4 are for 3 years . The interview at the embassy was more than simple . They didn't ask for anything other than the Petition and the Approval Notice . They didn't ask for my contract , my ECFMG certificate , my marriage license ...nothing . One thing that was probably important was that I am in a EU member country . In retrospect , I think my H1b was worth every penny .
