Hi, I'm having trouble getting a statement of need from the health ministry of my home country.. which may make it impossible for me to get the J1 visa does anyone know what will happen if I dont take up the position this time and re apply for H1 next time with step 3?.. will I be banned from subsequent matches by NRMP?

Do you mean you don't join the program this year? Well, the program will just offer the position to someone else and you can start from scratch the next year. You won't be banned from the Match unless you breach the agreement DELIBERATELY.
