Why in Carcinoid syndrome, a metastatic lesion to the liver is necessary ( when the tumors come from the intestine) to produce this syndrome (flushing, diarrhea , etc...).
Please help me with this. nod

as whatever substance that may cause symptoms would be cleared up when it drains via the portal venous system by the liver.
as a result, there are 2 conditions where you have carcinoid syndrome:

1. intestinal tumor + liver metastasis (the substance drains directly to the systemic vein)

2. tumors that drain directly to the systemic veins (not a small bowel tumor) like primary lung or ovarian carcinoid tumor
