In intrarenal problems/ATN, there is a concentrating defect, so why is the Urine Na MORE THAN 40. (spillage?) Whats the normal Urine Na by the way? Whats the best way to distinguish between Pre-renal and Intrarenal causes of Renal failure? -BUN/Cr Ratio? Normal: 10/1 Prerenal : >20 Intrarenal: <10 What are the other helpful signs?

Ill start and answer some: For prerenal--Fractional excretion of sodium less then 1 %. urine sodium less then 20, specific gravity greater then 1.020, many hyaline casts. I like the fracional excretion of sodium of less then 1%..

And Intrarenal/ATN: Urine Osm: <350 Urine Na: >40 (WHY?) Fractional excretion of Na >1% Urine Sediment: Full (brownish pigmented granular casts, epithelial casts) BUN/Cr Ratio <10 Specific gravity LOW Still wondering WHY the Urine Na would be >40.... Any ideas? Can someone explain this? Thanks.

guys, if I'm not wrong then in Intra renal cases ..the urine osm ls >350 & urine Na is high & in in pre renal cases ....the urin osm is <350 & this is b/c of low urin Na levels
