A 7-year-old child has had a high fever and a sore
throat for 2 days. Examination shows pharyngeal
erythema, a swollen right tonsil with a creamy
exudate, and painful right submandibular
lymphadenopathy. Throat culture on blood agar
yields numerous small β-hemolytic colonies that are
