A man with depression would have a face that's pissed off - so lets remember FACE PISS

F- Feelings of guilt, anger, sadness
A- appetite, weight-loss, diet
C- Concentration levels
E - Energy levels

P - Psychomotor disorders
I - Interests / hobbies
S - sleep / sexual problems
S - Suicidal ideation , Substance Abuse

Covers most of the special depression questions that must ask - not necessarily in the correct order. Never forget an important thing in depression - get the precipitating incident - allow the person to come up with it himself but if he does not and tries to avoid talking about it - then jst be direct yet friendly and ask "Mr. Ray, I know that all these questions are tiring and stressfull to answer, but as your physician and friend, I am very concerned about your well being and need all assistance to help me help you - You said you were perfectly fine 2 months ago ...what was that one thing that really made you stressed and sad ?" - this should bring it out - for example - a gay son .

dear doc,

can we voluntarily say "you said u were perfectly well 2 months ago...." even if the pt dint really say anything about it?

good luck.
