A 23 year old female comes to your office because of a solid mass in her right breast from 2 years ago. She also tells you that sometimes a bloody discharge appears on her right nipple. At physical examination you find a solid semimovable mass with some of contourn. At FNA, you find a diversity of atypic, malignant and normal cells. What is the best next step in the management of this patient? a) Chemotherapy alone b) Chemotherapy and Total mastectomy. c) Lumpectomy, Drainage of lymph nodes and Radiation therapy d) An excisional biopsy e) An steroractic needle biopsy then management f) Tamoxifen alone g) Lumpectomy, Drainage of lymph nodes, Radiation therapy and Ovarian ablation h) Ovarian ablation, Chemotherapy and Tamoxifen i) none of above

However, the age of the pt. speaks against this possibility. Malignant breast tumor are usually rare at this age. I would incline to chose between c & d c) Lumpectomy, Drainage of lymph nodes and Radiation therapy d) An excisional biopsy C is a better choice in my view. What is the correct answer ?

answer-c, Id give tamoxifen if the mass is estrogen sensitive.

i wil choose option c no matter what the age or history of size of lump bloody discharge & fna shows malignancy treatmant is lumpectomy axillary clearance and radiotherapy

y not go for an excisional biopsy to get more details before going to definitive therapy
