I get the feeling that the Kaplan simulated exam is just a bit tougher than the NBME - is it the most comparable to the actual exam? Any thoughts appreciated....

Neither one is similar to the real test. Kaplan is more picky, and both are more straight forward questions. But the point is, the NBME score you get does correlate with what your actual score will be.
