A woman during pregnancy affected with pulmonary emboli, what is your option for her treatment during pregnancy?
A. heparin at first then warfarin as maintenance therapy
B. heparin during all pregnacy period
C. heparin at first then asprin
D. filter installation in inferior vena cava
First of all warfarin is contraindicated in pregancy for being a teratogen. So the only chance of using warfarin is as maintainance therapy postpartum.
In the following reference from Medscape, heparin either unfractionated or low molecular weight is the preferred treatment as can be rapidly seen in Table 1. Aspirin is added to women with anti-phospholipid antibody syndrome.
Although that article is NOT about PE, that's the best I could find. So I'll go with option B because the question is so vague.
