4. A 21-year-old African American college student has had increasing fatigue over the past 3 weeks. Since an episode of cystitis treated with trimethoprim-sulfamethoxazole 3 weeks ago, he has been unable to keep up with his physical education classes. I think its D.....

As the reticulocytes count of 15% suggests that it could be due to chronic hemolysis, which is the most common cause of anemia in SSD.

TMP-SMZ therapy leads to depletion of folic acid, which may lead to megalocytic anemia (MCV >95). however, its not the case in given question...

Yeah, i think its G6PD.

The clues here are: 1. African and Mediterranean higher risks
2. Drugs that iduce hemolysis: dapsone, sulfamethoxazole, doxorubicin
3. Also infections produce hemolysis

he's also been following a vegetarian diet for 6 months, which means he could could have been eating lots of fava beans, thats another clue.
