how do u clincally differentiate torsion testes and acute epididymitis?

Epididimitis may have a discharge, UTI like picture, more common with fever.
Torsion comes with nausea, and more sudden pain.
The rest of symptoms look even more similar.

in epididymitis ..testis in normal position..(while in torsion it is elevated from its original position..)

right testicle is normaly lower than left, so the position of the torsed ball can not serve as the maker. I don't think one can definitely assess the position.

hi mdwannabe,

as kaplan says , life is different ...boards is different smiling face ...u may be a surgeon and may say the practical difficulty...but theoretically elevation is a point for diagnosing torsion...(may reach even inguinal canal)..also age group <30 (mainly prepubertal) for torsion
>30 goes for epididymitis...


also clinically elevation of testes (further manually) increases pain of torsion..lessens in epididymitis... :idea:

what abt the cremasteric ref'lex? is it present in epididymitis and absent in torsion?

and that's Prehn's sign u mentioned usmleasr!

elevation of testes deacreases pain in epididymitis, and not in torsion of testes

in torsiion cord is not tender , in epid. it is tender
