Hi!

Can someone please remind me of what happens to ST segments with MI?

If you care to add any hi-yield info which might be useful to remember about electrocardiograms for USMLE, that would be great too/

Thanks!

It elevates more than 2 mm off bease line. Diffrerent leads showing elevation will say the anatomical position of occlusion. All leads elevated suggests endocarditis.
Do u need the locations of MI's? I am not quite clear on what u want to know.

from Cardiology i can tell you there are two types of MIs:
1) "ST elev. MI" which is the typical case that indicates Hypoxia of the myocardium but NOT Necrosis yet, from here till signs of Necrosis show up (Deep Q,later ST depression, negative T), there is a time window during which the blockade should be openned. in other words before the appearance of Necrotic Q,etc.

2) "NON-ST elev. MI" is the second type that shows clinical signs of MI (angina> 20min.) but NO ST changes. according to Harrisons it is a result of an incomplete block.

for your info these two + Unstable Angina together are called
"Acute Coronary Syndrome"

now from the point of locating the MI you will see various ideas but this one is a logical one:

Ant. : I,avL,V1-V6 (if all chest leads are involved they refer to that as Extensive ant.)
Infer. : II,III,avF
lat.: V5-6
Post.: V1-2 (but a MIRROR image of typical T en dome/Rabbit figure)

hope that helped.

mdwannabe, are you sure about Endocarditis `cause i remember vaguely that if all leads or non-related leads show ST elev. that is a PERICARDITIS ??!! give me a clue
