Yes Amiodarone wld be the choice.....

V tach....Digoxin precipitates V tach(monomorphic)....u just can't give it.....depsite the fact that it fits well for Afib and the CHF

CUz Amiodarone is just a beautiful drug....hehehe

It is virtually always correct......It is the broad spectrum antiarrythmic and now
It is now even recommended by the American Heart Association for the treatment of VFib as first drug instead of our famous Lidocaine :shock: (surprise no)
yeah ...but since it is only given one ...then Lidocaine would come after the next shock.......

isnt digoxin indicated in Afib to decrease the ventricular rate?
and also in CHF?
and isnt amiodarone for atrial and ventricular ectopcs?
then in rinks question on the 14 of July, isnt digoxin the right choice? :?:

Yes absouletly correct.....Digoxin is indicated in A Fib with CHF but this patient has V Tach and
V tach....Digoxin precipitates V tach(monomorphic)....u just can't give it.....depsite the fact that it fits well for Afib and the CHF


So the whole dilemma here is the V tach.....
and yeah Amiodarone is given to treat significant ventricular ectopics... protract it a bit more.... it becomes excellent for V tach

Plus it is well known to improve ventricular function......

So It is just tooo perfect an agent to refuse....

One thing....Amiodarone is like the Mecca of Antiarrhytmics...i doubt u shld ever say no to it...unless they give u a patient with some thyroid condition..... or taking radioactive iodine..... then u better not choose it

I THINK THE COMBINATION OF THESE THREE SYMPTOMS i.e. ATRIAL FIB., CHF AND V. TACH IS TYPICALLY PRESENT IN ONLY PATIENTS OF LONG STANDING THYROID CONDITIONS i.e. HYERTHYROIDISM.
AND WE ALL KNOW WELL THAT AMIODARONE IS CONTRAINDICATED IN THYROID DISORDERS. SO DIGOXIN MAY BE THE RIGHT ANSWER IN THIS SETTING. WELL THINK ABOUT IT.

Hello Shirrish...
this is a question form step 1 as Rinks has mentioned so there is no place for assuming what the patient might be typically predisposed to ....

He has the 3 things there.....
and as i see it ....V tach is serious enough. ...and u need to treat it..
Have u ever heard of one treating V tach with digoxin :!: :P i don't think so.....
Plus u can ever argue about that ..... treat the patinets thyrotoxicosis and give him or her Amiodarone still for its broadspectrum action
