i think if we have to choose 1 specific it should be anti ds DNA as it is positive in 60%
while anti -SM is positive in 25-30%

Dimps, if we have to choose single most specific then anti Sm are most specific because they are not found in any other disease.

Well, what I know is that the only Ab which has 100% specificity is Anti Sm antibody. Anti ds DNA has a high specificity for SLE too, but not 100%.
I remember from one of kinda group discussions, a friend of mine who took IM boards talking about this. He got this q in IM boards.

yes anti sm and antidsDNA most specific
ANA ..is best screening test .
:arrow: so for a case of SLE we do:
1. ANA
2.Anti sm and antidsDNA
3. antiphospholipid antibodies
4. complement levels( CH50,C3,C4)
