32yrs old lady, known pt of hyperthyroidism goes through an emergency surgical procedure. After surgery she is found to be extremely restless and irritable. She is confused and 'not in her senses'. Her pulse is sinus 120/min, temperature is 103 C. WOF is not an option for her treatment?

a) propylthiouracil
b) hydrocortisone
c) salicylates
d) parental fluids
e) beta blockers

it is a condition called thyroid storm
and its treatment include
hydration
prppylthiouracil 100mg every 2hr
iodine
b blockers
glucocorticoid
answer is --salicylates


DIAGNOSIS AND TREATMENT Thyroid storm is diagnosed on the basis of symptoms. Although laboratory tests indicate thyrotoxicosis (by elevated thyroid hormone levels) there is nothing to distinguish thyroid storm from ordinary hyperthyroidism.

Treatment should never be delayed. Therapy is directed at supportive care and prompt reduction of thyroid hormone. Drugs to reduce fever (antipyretics) are indicated, but aspirin should be avoided, as it is known to increase thyroid hormone levels. ATD's and strong iodine solution (Lugol's solution) given an hour later (to prevent excess iodine causing hormone production) are administered to reduce thyroid hormone synthesis and release from the thyroid. Beta blockers are also given to control symptoms.
