Hey Doctors!

1)What's the most common diarrhea bug in US adults

2)Why is HBcAg not measured.

1) Virus: Rota and Adenovirus
Bacteria: Staphylo and salmonella (my guess)

HBc Ag is located inside cell where virus is replicating, so it is impossible to detect it in plasma. Hence, HBcAB is always positive even in "window" period.

1. salmonella and shigella are the two most prevalent bacteria that cause diarrhea; as to Rota virus, it mainly cause problem in young children, not adults.
2. HBc Ag is located inside viral core. it is not exposed and therefore is undetectable.

Thanks for the replies everyone.

For 1), I thought it might be E.coli, but I guess this is only for travelling adults. What do you think?

2)I understand that HBcAg is in the core, but I thought HBeAg was too--yet it is measured. I guess I will just accept that it is really, really, really deep inside the core, and is not worth/good idea to measure.

Campylobacter-(the curved,flagellated,fastidious microaerophilic negative rod) is the most common cause of bloody diarrhea in a healthy individual!

I would still stay shigella and salmonella for the 1st q. I think E. coli is the leading cause of UTI, not diarrhea.

HBeAG is the infective stage/reperuduction stage particle. it leaves the cell and is detecible in plasma. It is a split product of the viral polyprotein.

I think its either shigella or campylobacter......I think I'll go with campylobacter.

Jawetz and Kaplan both say Campylobacter is more common than salmonella and shigella together!

Yes, it is Campylobacter. The problem with Campy is it's harder to grow, and must be plated on special plates and incubated in a microaerophilic atmosphere at 42 degrees C. We use special Campy paks with a generator inside. We don't open the Campy plates for two days.

We see a lot more Salmonella than Shigella.
