Q. a patient presents one wk after appendicectomy with high fever and a red fluctuating swelling on the medial aspect of the wound. which is the most approp Mx?
a. release the suture to prevent sinus formation
b. drain the abscess ( I&D)
c. give broad spectrum abiotics
d. Abd CT
e. Abd Xray

agree :-) A/B will never work for abcess...couse there is no tissue there to respond ... Drain it well.

I also agree, but the clear reason to drain any (true) abscess after having made the appropriate (presumed and in this case probable) dx, and not provide AB as the first choice Tx would be that an abscess by definition has contents which are necrotic and devoid of blood supply (circulation), while AB arrive by the circulation. Even the wall, which is fibrous because it is the body's response to seal off the toxins formed in the abscess, has very little blood supply.
