Hey gang!

Anyone remember what bilateral otitis media is nearly pathognomonic for?

Wegener's...Right?? If that's correct though, could somebody please help me out with the pathophys on that? I mean, what I know about W's is that its triad is upper & lower airways involved + kidneys in a "necrotizing vasculitis" (I don't *really* know what the heck that means) .

Anyone throw me a life raft? I'll post this in path too just in case (hope I'm not breaking forum rules by doing that..)

I think it is due to obstruction. Remember, this applies to adults mainly. Chldren get bilateral OM all the time.

Necrotizing vasculitis is mainly just term for diseases that knock off small vessels, due to immune complex deposits leading to fibrinoid necrosis. The antibody in wegener's is c-ANCA, the Rx is cyclophosphamide and prednisone, and the s/e from cyclophosphamide is hemorrhagic cystitis and transitional cell CA of the bladder. It leads to RPGN, much like Goodpasture's, but is often not directly preceded by hemoptysis.

wegners involves nasopharynx, sinuses and kidneys...i remember saddle nose deformity and septal perforation asso. with it....
as idiopathic told abt c ANCA
Tx of hmgic cystitis due to cyclophosphamide is mesna(just to revise)
