-elevation of BP >220 systolic or >120 diastolic could be either malignant hypertension or urgent hypertension.

-to diagnose malignant hypertension papilledema should be present......if no papilledema this is urgent hypertension.

-malignant hypertension should be admitted to the ICU.....urgent hypertension doesnot need ICU admission.

treatment of hypertension:

stage 1 "systolic 140-159 or diastolic 90-99" thiazides

stage2 "systolic>159 or diastolic >99" 2 drug combination : thiazide+ACE or BB

this is if no other comorbidities present .

-cardiac enzymes may be normal within 4-12 hrs from the start of MI .....so the diagnosis of MI cant be excluded based on a normal initial cardiac enzymes.

Stationary travel for more than 4 hours increases the risk of venous thromboembolism 2-fold, even several weeks beyond the time of travel. Clinical risk factors included previous thromboembolism and varicose veins .
