Hi Nuprin! How are you doing?
I have another Q for you if you don't mind!
This Hardy Weinberg Qs????????? drives me crazy! I read many times and still get the Qs wrong on that!
Please if you don't mind help me to clear this? I need good explanation of the whole thing! I am sure there are some people also having hard time figuring this out! So please help us! Where can i practice more Qs on this?

I wasn't talking about starling forces, I was talking about clearance, effective renal plasma flow, about calculating plasma or urine osmolarity when certain dates are given like the initial osmolarity, and then after the person takes in some salt or water to recalculate the final osmolarity..This was not a clear exemple, I was just trying to see if you had questions like these,, and if you had a good source to do exercises conneected to renal clearence and excretion..thanks

by going to lab, i meant going to the labs that were required wth my coursework. i always hated micro and neuro and even anatomy lab, but even the short time i spent there i found i retained quite a bit that i could recall during my exam.

krampus, i don't think i had too many of those types of questions, but to be honest, it's kinda a blur now. i doubt, but can't guarantee, you'll get more than a couple of these bc there's not that much straight up phys. definitely listen to goljan for help with that stuff--not so much the calculations, but the concepts and what to do--that's the kinda stuff you'll see a lot of on the boards. oh, and check brs phys for some qs. have you already?

elm, hardy weinberg... hmm... how best to explain? please correct me if i'm wrong:

1) think of the algebra formulas by themselves and really understand them: (p+q)^2=p^2 + 2pq + q^2=1=p+q (e.g. in order for p+q squared to equal 1, p+q has to equal 1, too)

2) p is a normal allele and q is a disease allele

2) think of p^2 as incidence of homozygous normal people (because if both of your alleles are normal, you're p^2) and q^2 and homozygous diseased people.

3) think of 2pq as # of heterozygotes

okay, now an example:
say they tell you that the incidence of a disease is 1/625.
this means q^2 = 1/625
that means q = 1/25
if you plug that in p+q=1, you can figure out that p is 24/25 (which you can just round up to 1 when the disease is pretty uncommon)
now just plug into the formula: p^2 + 2pq + q^2 = 1
the incidence of heterozygotes is 2pq-->2 * 1 (actually p=24/25) * 1/25 (q)= 2/25 = 4/50 = 8/100 (etc., change fraction to whatever they're looking for) = 8%

does this make sense? the key is, 1) know that the incidence is q squared. and 2) take the square root in order to plug into the 2pq heterozygote part.

also, remember the hardy weinberg stipulations on p. 145 of FA.

if they tell you the incidence of an x-linked recessive disease, know that that = the # of heterozygotes = 2pq bc you only need one allele (heterozygote) to have the disease.

i think goljan has a couple of these practice qs, but other than that, i'm not sure where to look...

Can you please tell me how did you organize your preparation for step 1? I know the books you used but what about the time you took to prepare and sources of MCQs? I am planning to take my exam by the end of November, Do you think 4 months is enough time to get ready?

gianna--sorry it's taken so long to write back--i don't think i saw your message before! or have i already responded? hmm... i think i'm going crazy! i gave myself 5 weeks to prepare. i got high yield-type books for all subjects and set a certain # of days to study each one, depending on how much material needed to be covered and how important it was for the usmle. for example, anatomy, histo, embryo, and neuro got 1 day (i think... i'm starting to forget), but physio got 4, pharm got 5, and path got 6 days. pm me with your address if you want me to send you a link to several different schedules.

if i could do it again, i'd take one more week so i could both start and end with goljan audio. i would have spent less time on robbins review (too thorough unless covering a weak subject), and given myself more time for first aid revision.

as for whether 4 mos is enough or not, i can't say. how long has it been since taking the courses? how much time can you devote to studying each day? have you assessed yourself right now so you know how much you need to improve?

okay, so to end an era, i finally got my scores today. i feel a little strange about posting my scores online, esp if there are any identifying characteristics to tie me to my real-time persona, but i guess since i've dished out so much advice it's about time i 'fessed up.

so i got a 235/95. not the 99 i was shooting for, but i suppose not such a bad score. my nbme taken five days previous was 590, which almost exactly correlates with the correlation i posted a few times: acc. to that, a 600=236. so the two digit score isn't quite as generous, but the three digit is right on the money. my best subjects (the ones with the little stars next to them) were renal(!? a total surprise for me), skin and connective tissue (weird, huh? also my best in qbank), and GI. my worst were nutrition and respiratory.

so if i had to do it all over again, knowing what i know now, here's my advice, which pretty much corresponds with the advice i've been giving. if you haven't yet started m2 year, study hard. i didn't, and i had to pay for it by learning everything from scratch in those five weeks i'd alotted for my step 1 studies. studying hard m2 year and learning it the first time would have saved me a lot of time and effort. do gojlan twice, once before starting studies and once to conclude. i took 5 weeks to study but wish i had taken 6 so i could have started with goljan. secondly, finish ALL of qbank. i got 90% done, half in the final week. those last few days i learned a LOT from qbank, so i wish i had finished the final 10%. third of all, give yourself enough time for revision of ALL of first aid, and memorize every last word of it. i only had time to revise behsci, biochem, anatomy, and micro, and this was all the night before. the stupid stuff i missed on test day were things that came right out of first aid. fourth of all, don't waste too much time on a billion review books. pick one per subject to help you understand, and spend the rest of your time memorizing the corresponding first aid section. i wish i hadn't spent as much time as i did doing robbins review and had rather spent that time memorizing first aid or sleeping, which leads me to my last and final point: GET LOTS OF SLEEP the night before. the test will tax your reasoning ability, which will function much more efficiently with a good night's sleep.

i want to thank each and every one of you once again for your continuing support and prayers. it really meant a lot to me, and without those, i wouldn't have made it. best of luck to all of you in your future endeavors and careers. you're a great bunch of people, and i would be honored to work with any of you in the future. for those of you still about to take this test, press on to your final goal--with hard work and determination and tenacity and faith, you will succeed. as always, i'll be happy to answer any questions you have, but i'm starting clinicals shortly and won't have as much time to hang out here, so don't be offended if it takes me longer to respond.

oh, one more thing, bf i get asked. this is all from memory, so forgive me if i miss some details. i didn't do the nbme 150 qs. actually, i did do one section a week or so later, and i got like 45. i got 590 on nbme 1, taken 5 days previous, and 530 on form 2 taken two-three weeks later (don't ask). i got a 71% on kaplan half length diagnostic taken two weeks prior and 72% on kaplan full length, taken one week prior. i started random qbank with around 60-65, went up to 65-70 bf path and pharm, and was getting 75-85% in the final days for an avg of around 75-80% with repeated questions and 70% overall (not including repeated questions, i think). i did some subject based questions in the beginning but stopped bc i thought they weren't going to be representative (was getting around 75-90%).

I have to admit, that all this time you were right and I was wrong about the score correlations. I was SO envious of you, being so sure you would smash the test (and you did). I was scared until I unfolded the score report from the envelope. You worked hard, and you got what you deserved. A stellar score by any means. Congratulations!!!!
