For a pregnant lady who is Rh negative , before giving her RhoGAM we do indirect coombs test at 28 weeks .Why ? and if the result gives positive indirect coombs test ,what is next step ?

First fetal-maternal blood contact occurs at 28th wk. Indirect coomb is done to look for any sensitization of Rh-ve mother before this period. If mother is not sensitized already, antiD is given. If coomb is positive then anti-D has no effect as there are already Abs present. The fetus is monitored closely for fetal distress. In case of hemolysis, intra-uterine transfusions can be given. OR delivered early at 32wks after giving steroids?? am I going the right way ??

Agree with the above, plus: Blueprints has a nice, sequecentiel approach: 1: Do antibody titers. If they exceed 1:16, go on to 2 2: Do a photospectometry (or something like that) on chord blood to look for bilirubin. If that's high go to 3 3: Transfuse or induce
