hi bobby,
clincally ,fever,costovertebral angle tenderness should point to kidney infection...and suprapubic tenderness and urgency to cystitis..
if u ask inv...then simple test will be urine analysis, APN will have WBC casts....while cystitis will have only WBCS (not casts)
