According to kaplan lecture notes, you see increased serum proteins but NORMAL albumin, however, in a kaplan question bank explanation, it said you see DECREASED albumin. Can someone elaborate please??? :?

in my opinion, albumin shouldnt decrease, however i ll welcome any comments for other buddies

You only have a finite amount of protein...and you will lose protein in the urine, INCLUDING albumin. So, you use up your protein making IgG and have less to make albumin.

Hypoalbuminemia is an adverse prognostic factor for multiple myeloma. I suppose albumin would start leaking if there was glomerular damage, which may occur secondary to multiple myeloma. Isn't the serum protein elevated in multiple myeloma? I don't think that the body has a "finite amount of protein."

i think synthesis of protein is not limited, so if supply of essentials of synthesis is present, it will continue. i think u r reffering to renal loss in myeloma nephrosis, m i rite? yes that can be a cause of low albumin

Remember, visceral protein (albumin, apolipoproteins and IgG) depend on protein intake, whereas muscle can be made from decent carbohydrate intake. I think this is the difference between kwashiorkor and marasmus. The kwashiorkor child gets enough calories, but not enough protein, and so his visceral protein is low. I think (opinion) that this could also happen in hyper-producing states (i.e. multiple myeloma). However, the answer seems to be nephropathy from Bence-Jones proteins (25% of MM cases). :lol:
