CD MARKERS BY FLOW CYTOMETRY AND I THINK CD5 AND CD27 ARE MORE SPECIFIC FOR SMALL CELL LEUK.

both are CD 19, 20 & 5 positive and CD10 negetive. i dont know about CD27. peekay, would u plz eloborate a bit or more or give me reference?

I know one way to differentiate aside from markers is that CLL is leukemia so patients will have blood findings, and small is a lymphoma so patients will have lymph node findings.

well rida that seems reasonable but lymphomas are also known for "leukemic" (blood) involvement and leukemias for lymph node involvement. the distinction u ve made is applicable to very early stages.

The latest WHO classification on lymphoma lists this disease as CLL/SLL meaning there is no difference. The cells have exactly the same phenotype. However, when SLL is leukemic, it's called CLL. If you have a small lymphocytic lesion (often starts in retroperitoneum, not in the nodes, and goes to the spleen), which is CD5 (usually a T-cell marker) positive besides B-cell markers CD19 and CD20, then it is either CLL or Mantle cell lymphoma. These two are seperated by presence of CD23 (positive in CLL, negative in Mantle) or Bcl1 (positive in Mantle, but negative in CLL). Morphologivally Mantle cells are more cleaved and irregular, while CLL cells look just like normal small lymphocyte. Also Mantle cell lymphoma comes from the lymphnode - follicle mantle zone, e.g. starts in lymph nodes.
