Is there a contraindication to using adenosine to Rx PSVT in a patient who has WPW?

  #2

I checked Harrison and in Kaplan- I didnt see any contraindications although it wasnt mentioned specifically for PSVT in WPW.
Seems like its OK since it decreases SA and AV nodal activity and increases AV nodal refractory period[Gi coupled decrease in cAMP] the half life is less then 30 seconds. causes membrane hyperpolarization
But Idio I appreciate your ambition and your girth but Im pretty sure this is more for Step
II ( Step I as you probably know will cover clinically rate, rhythm, murmurs, blocks)** also its antagonized by theophylline. Hope this helps a little.

Goljan has it listed on our high-yield sheet he gave us in class: "Know Rx of PSVT with adenosine and association with WPW"

When using adenosine you need next to you a defibrilator because there is a risk for developing AF. For this reason one should not use adenosine in a patient with WPW+PSVT.
