The elevated homocysteine levels may inhibit the activity of thrombomodulin and lead to platelet dysfunction, with increased levels of thromboxane A2. Moreover, the endothelial cells are damaged, perhaps creating a prothrombotic surface.

upregulation of synthesis of factor VIII and V and downregulation of antithrombotic factors.
