Which of the following is correct about HIV in pregnancy?

a. Pregnancy accelerates maternal progression from HIV to AIDS
b. Mode of delievery has no impact on maternal neonatal transmission of HIV
c. Breast-feeding does not increase neonatal risk of becoming HIV
d. Noenates can be protected from HIV by passive immunization at birth
e. Rapidity of disease progression is the same in mother and neonate

a is the correcy answer.autoimmune diseases improve and immunodef states get worse during pregnancy.

b is totally wrong.Infact one of the ways to prevent vertical transmission of HIV is elective cs.Of course it is only indicated in pregnant women whose HIV disease is poorly controoled with ART.Another way of prevention is Vit A supplementation.but the cornerstone of prevention is ART.Zidovudine starting at 14wks to term to delivery and to the neonate postpartally.
In a pregnant woman whose cd4 is below 350 or viral load above 55000 three drug regimen should be started like any other patient.Any of the drugs can be given exept efavirenz which might have teratogenecity.
