:?: A 40 year old man came to see a his doctor after he lost his conscience three times in a month. He reports that all three episodes happened when exercising on his Bowflex device and started with muscular pain in his right arm followed by problems with his voice and vertigo before falling unconscious. What is the most likely diagnostic? Sorry, no options!

Wow! That was fast ! Subclavian steal syndrome is the answer. The obstruction of subclavian artery proximal :!: to the origin of vertebral artery will result in shunting of the blood from the cerebral circulation to the exercising arm. The result is arm claudication plus neurological symptoms. Bypass is the treatment.
