I dont like including alcohol on any inhibitor list, since it induces its own metabolism. Inhibition occurs through indirect destruction of the kidney and not through down-regulation of cyp450 enzymes.

I couldnt believe it, but the quinidine thing is true...it really is an inhibitor. Contrary to my own understanding.

these are the MOST important. it will be impossible to remember everything that inhibits/induces the cyp450 system, but these are truly the HY ones.

idiopathic, could you please explain why quinidine is inhibitor of P-450? or if you can direct me to other posts :roll: where you've already explained it?

LANGE Pharmacology (Katzung/Trevor) credits it as an inhibitor, most notably of cyp2D6. Also, if you search on the internet, you will find several sites that list it as an inhibitor. Obviously just a mistake that caught on...even FA counts it as an inducer (our Pharmacology prof did the same thing).

Ok, but since there is argument about quinidine, I don't think it will appear on the test. :wink:

The funny thing is, there doesnt seem to be any argument about it, it is an inhibitor. But FA, and everyone who reads it, thinks it is an inducer. I agree with you that it probably wont be on the test, but you never know.

alcohol acts on CY p450 in a different manner-- acute consumption INHIBITS while chronic alcohol ingestion INDUCES the enzyme. some questions test this understanding--so thought it would be of some help...

Chronic induces the enzyme in order to increase the rate of metabolism of alcohol. Chronic alcoholics have induction of the CYP2E1 system (part of the P450 system), which leads to greater toleration of alcohol.

thanks sakaki , for the same reason acetaminophen toxicity is enhanced in chronic alcoholics--induction of CYP 450(normally a minor pathway in alcohol metabolism) leads to increased production of a toxic metabolite benzoquinoneimine that leads to hepatic necrosis.benzoquinoneimine is inactivated by glutathione the stores of which is finite.

CYP 1A inhibitor Quinolones
CYP 2C9 genetic polymorphism (varied metabolism)
CYP 2D6 inhibited by Haloperidol and quinidine
CYP 3A4 (The most important one ) CIMETIDINE , Azoles(ketoconazole etc), Macrolides(Erythromyin) and Acute Alcohol binge.

This could serve as a comprehensive HY list. Hope it helps
