Which of the following procedures or clinical signs is most specific for the diagnosis of infectious mononucleosis caused by the Epstein-Barr virus?

A. Laboratory diagnosis is based on the presence of "atypical
lymphocytes" and EBV-specific antibody
B. Growth in tissue culture cells
C. Heterophile antibodies in serum
D. Lymphadenopathy and splenomegaly on physical examination
E. B-cell lymphocyte proliferation

specificity based on heterophile antibodies, as many things cause atypical lymphocytosis.

I agree with Idiopathic--Heterophile antibody is the gold standard/ sometimes the Monospot test. [I see that these test are normal in patients without disease]
The atypical lymphs or "Downey Cells" is only some guy trying to get his name in history.

The answer given was B. Which I think is totally wrong. I would go with C. Heterophile anitbodies.
Does anybody have any more information abt this?

Yeah, initially I picked C because it specifically says MONO caused by EBV.

If they didn't specify that, then C could not be the right answer b/c if the patient has a +heterophile activity, the cause is EBV. If it's -, think about CMV, HIV, Toxo. So, heterophility just helps you determine the cause of Mono, not whether the pt has mono or not.

I remember the kaplan professor mentioned that dx of Mono is by detecting IgM :?

answer is a as the question refers for specificity of the test, not sensitivity.

apart from the atypical lymphocyte, could EBV-specific IgM or IgG Ab be the answer?
