After one episode of mania, pt has been on on lithium 3y. He is asumptomatic now, would like to stop the medication, but his mother apprehensive and want continue a longer period of time. What would be the most appropriate intervention in this pt? a. Taper and discontinue lithium b. Listen to the mother and continue lithium c. Order lithium levels d. Continue lithium at the same dose life-long

A. Taper and discontinue lithium ( The majority of patients do not experience withdrawal symptoms or rebound phenomenon upon cessation of long-term lithium therapy. In view of the occasional reports of sudden relapses occurring with abrupt discontinuation, gradual discontinuation is recommended unless abrupt withdrawal is necessary because of toxicity. - RXMED.com)

You are perfectly right. For the initial episode of mania, treatment duration with lithim is around one 1y; if repeated episodes happen 3 times or more, then lithium should continue life long. Thanks. (Source: uworld qbank)
