in left vent outflow obstruction if we decrease preload, it results in increased outflow obsturction. why??

The increased preload helps keep the mitral valve leaflet 'pushed back', away from the thickened IV septum. The longer good blood flow is surging past the aortic valve (i.e. increased prelod), the less the mitral valve gets sucked up against the septum, obstructing flow.

I remember that this is exactly the opposite of the aortic stenosis murmur, which increases with increased preload.

Note: for anyone who doesnt know, this is hypertrophic obstructive cardiomyopathy (HOCM) or idiopathic hypertrophic subaortic stenosis (IHSS)...same thing with two names.
