when collaterals develop (as a result of angiogenesis) they "steal" the blood supply away from the necrotic area of the heart, so that area never gets adequate blood supply. also the already present arteries that are healthy will vasodilate better than the vessels of the affected area, so they too will 'steal' blood away.

hope this helps

one other thing i remembered was that drugs like dipyramidole vasodilate all the coronary arteries---but as you know the healthy arteries will be able to vasodilate better than the vessels in the necrotic area, so this decreases the flow of blood to ischemic area and this further hypoperfusion worsens the injury.
