What are the main differences between Marasmus and Kwashiorkor.(Although not seen in the U.S.)

Marasmus = calorie deficiency. visceral protein intact (i.e. albumin), but somatic protein deficient (muscle). HUNGRY.

Kwashiorkor = protein deficiency w/adequate calories. somatic protein intact but visceral protein deficient. ascites due to low oncotic pressure. huge fatty liver due to decreased synthesis of VLDL. NOT HUNGRY.

Marasmus- alertness broomstick extremities and as you said loss of muscle mass

Kwashiorkor-diarrhea(due to loss of brush border enzymes) depressed
immune response, flaky paint dermatitis, apathy, and the 2Ps protruberant abdomen(fatty liver due to excess carbos and decrease apolipoprotein synthesis) and as you say pitting edema due to hypoalbuminemia
*what sticks out in this case is here is someone with a "fat belly" actually starving. (as said normal muscle mass)
