39 year old man comes to the ER with a high fever. He reports similar episondes 2 and 4 days ago and reveals that he just returned from a trip from Panama. The only physical finding is splenomegaly. What is the diagnosis?

A) Amebiasis
B) Leishmanisasis
C) Malaria
D) Toxoplasmosis
E) Trichinosis

Malaria...fever pattern is key. Could also be recueent fever of type Borrelia recurrentis (not sure about the pattern...I think it is weeks rather than days).

Don't have answer but my guess is also malaria. Whereami i think its malarica because of the cyclic pattern of fever as idiopathic pointed out. B is also a good guess, but i think the fever points more towards malaria, i could be wrong too of course.

yes, i also think is malaria. the paroxysms are every 44-48 hrs. (except when etiologic agent is Plasmodium malariae, then it is every 72 hrs.)
In Leishmania donovani (visceral or kala-azar) there is also fever peaks but they are two or three a day.
I just had a test on this, and it was very hard. Hope i did fine.
