A 16-month old is taken to the ER after falling while learning to walk. The toddler has an enlarging, swollen bruise on his forehead, which is now over 5 cm across. The parents say the bruise is noticeably larger than it was when they entered the ER an hour ago. A blood sample is drawn, and the child oozes blood @ the puncture site for 25 minutes. Clotting studies on the blood sample show a prolonged PTT ans a normal PT. Follow-up studies show very low levels of factor VIII. Which of the following is the most likely diagnosis? 1. Hempphilia A 2. Von Willebrand disease The answer is 1, but I disagree, based on von Willebrand disease (vWD) is thought to be the most common hereditary bleeding disorder, affecting up to 3% of the U.S. population, meanwhile Hempphilia A is much rare. Both of them share same features listed in the above Q. Why choose Hempphilia A?

yea, i go with you! and moreover in hemophiliacs the BT tends to be normal and this child has been oozing for 25 mins. With this much info it sounds more of vWF ds. please clarify, why the answer is hemophilia, if is so?

The answer just explained what is Hemophilia A and its clinical manifestations. Basically, nothing. That's why I posted it.

Ans is 1 becas level of factor viii is very low and vWd is most commonly associated with mucosal bleed like Epistaxis, menorrhagia etc.In this Q level of vWd also has to be there along with BT.
