diagnosis in 25 yr old female with proximal muscle pain and weekness and IBS :? and tenerness on palpation of muscles and normal joint motion range?
any clues to the diagnosis?

Polymyositis :?:

possibly primary idiopathic polymyositis.Only exception is inclusion body myositis where distal muscles r involved.

when I first read the question I was also for polymayositis , but after some research on emedicine I think it is fibromyalgia as it pesents at early age like this one and associated problems include IBD
* Other frequent symptoms include the following:

o Cognitive difficulties with attention and memory

o Weight fluctuations

o Allergic symptoms (eg, nasal congestion) and hypersensitivity to environmental stimuli (eg, odors, bright lights, loud noises) and medications

o A series of regional pains, including noncardiac chest pain, dyspepsia, headache, abdominal cramping (irritable bowel syndrome), temporomandibular pain, chronic pelvic pain

o Syncope

o Shortness of breath

o Urinary frequency and urgency

Yes IBS and fibromyalgia are classically paired.

I bet on polymiosytis because the diagnosis of fibromyalgia requires a history of a least three months of widespread pain, and pain and tenderness in at least 11 of 18 tender-point sites:
Neck ,Shoulders ,Chest ,Rib cage ,Lower back ,Thighs ,Knees ,Arms (elbows) ,Buttocks . Here is very clare" proximal muscle pain". I don't konw.

yes i too would go for polymyositis...
fibromyalgia though paired with IBS but i dont think there is any muscle tenderness and predisposition for proximal ms....there is widespread pain,ms stiffness more in morning,fatigue,sleep disorder,tm jt dysfxn synd,ibs and all those mentioned above...
can anyone clear this one...
