A 26 year old female is referred with intermittent diarrhoea present for many years. She states that her weight has been steady but describes watery motions up to six stools per day and has also noted abdominal discomfort with bloating. She has not been aware of any blood in the motions or melaena. She describes no other medical history and denies taking any medication.

Investigations show:
rest normal
Corrected calcium 2.2 mmol/L (2.2-2.6)

What is the most likely diagnosis?



1 ) Crohn's disease
2 ) intestinal tuberculosis
3 ) lactose intolerane
4 ) laxative abuse
5 ) microscopic colitis


























All her parameters are normal except calcium which is at the lower limit of normal. Calcium in the gut normally gets absorbed in the large bowel and laxative abuse causes lower bowel hurry causing decreased absorption of calcium. Lactose intolerance is unlikely because of the duration and age. Any form of colitis and other malabsorpion syndromes like ileocaecal tuberculosis causes raised CRP and decreased albumin with other blood abnormalities such as anaemia.
