Hello and thanks in advance!

I'm an IMG from the Philippines currently taking care of my ECFMG requirements. Here's my dilemma: I was hell-bent on getting a neurosurgery residency in the US and was fortunate enough to be offered a research fellowship at a university hospital. I spoke extensively with the PD and he has bluntly told me that having extensive US experience (in my case research instead of clinical because of restrictions) would be the only possible way to get a neurosurgery position (at his or another's program) because it is so competitive. My problem now is that I will only get a J1 visa for my research work, which explicitly states that it is valid only for 3 years. What is not clear to me is if I have to go home after my work is done (like a clinical J1 visa) or will I be immediately able to apply for a residency position again?

If I am able to stay, is it wise to get a J1 for neurosurgery given that it will take a longer time than say IM or Peds (6 years), at which point I will no longer have time for a subspec fellowship (ave. 2 years extra)? Will I be able to get a waiver as a neurosurgeon or is it strictly limited to primary care? Should I work towards getting an H1 even though you have said that fellowships will not sponsor an H1?

Last bunch... :lol: For IMGs who go into IM, will the same J1 be valid if after your IM you want to go into cardio, etc.? Any specific restrictions to this like having to get a fellowship at the same institution that sponsored your J1 initially? OR...do you need to do your waiver first, then reapply for a fellowship?

I apologize for all the questions! I know I should probably get a lawyer but I wanted your opinion/s first. Thanks again!!!

"Duration of Participation
The duration of participation for J-1 physicians in graduate medical education is the "time typically required" to complete the program. This generally refers to the minimum number of training years required by an American Board of Medical Specialties (ABMS) member board for specialty or subspecialty certification and/or the accredited length of training as defined by the Accreditation Council for Graduate Medical Education (ACGME). Duration is further limited to seven years, provided that the J-1 physician is advancing in an approved program of graduate medical education or training. The J-1 visa, however, does not entitle J-1 physicians to seven years of sponsorship. J-1 physicians are encouraged to consult with ECFMG regarding proposed educational pathways and sponsorship eligibility. "

It's best if you do your residency and fellowship back to back - it's when your application is the strongest. Has it been done where you do your residency, find a greencard waiver and come back to do fellowship? Yes, it has been done. In the cases that I know about, these individuals were stellar physicians and usually Chief Medical Residents in the program. Impossible, nope.

Hello and Thanx in advance.

I am from India and hell-bent on getting into Neurosurg too. Was just wondering what were the requirements for getting the research fellowship and what were your qualifications. I myself have just completed my MBBS and am doin my one year of internship. I have as yet to give my USMLE steps and was wondering if research fellowships were hard to come by. Where have you been offered the fellowship and how did you apply. Any pre-requisite exams b4 that?

Any help will be greatly appreciated.
