Very good question! Stress test has a good predictive value (+ or -) when the pretest probability of CAD is medium , i.e. not high and not low . In a patient with high pretest probability of CAD e.g. a diabetic with HTN and typical chest pain the first step is Angiography . In a patient who is asymptomatic or with low pretest probabiltiy , e.g. a young female with atypical chest pain neither Stress test nor Angio is indicated In a patient between two above , i.e medium pretest probability the first best next step is stress test and if it is positive you can preoceed with Agio. I suppose UW step 2 CK has a few good Qs in this regard !
