what kind of hypersensitivity is it ?
at 1st i had HS 3 but i think somewhre in goljan notes it said HS 1 and HS 4 ?

Anyways if sum1 can clarify, i would appreciate it.

brilliant Q.........significant points are: eosinophilia present in few of thepatients, penicillin is involved, it takes 5days to 5 weeks for showing up, shows up alongwith rash and fever!

But we can open our minds and learn a few things: Since theres a strong relationship with eosinophilia, type 1 might have a role!

the cool point is that the final work to prove any one of these HS reactions is still no where close, however latest work favours a cell-mediated immunity as the probable cause (so Tcell or type 4 reaction). at our level, we know that drugs, 4instance penicillin, form hapten by combining e the glomerular basement membrane. And then the Ab come into action! So type 2! goodpasteurs as another cause of interstitial nephritis (obviously not drug induced, lol) favours this view!

Type 3, very difficult to say anything in favour of this! SLE(type3) normally causes nephritic........

SO we can say its a combo of type 1,2 n 4, although you rightly pointed out that goljan called it a combo of Type 1 and 4!

thnx for clarification man, i thought it was HS 3 also because hypersensitivity penumonitis is type 3 so i figured y not this also.

oh and that hapten thing i dnt know i think thats with hemolytic anemia HS 2 but i dnt know if its with Intestitial nephritis also. Hmm but thats kinda detail, i havent heard one question on it except what drug is patient taking. I find it hard for them asking what kind of hypersensititivity because they will have to take most of the hypersensitivty out of the list. But then again, they can ask why the anemia HS 2 , rash HS 1 , nephritis 4 ..


i wish i can use 3 also lol because then i have this useless info about it not being 3 !!

hey goljan has explained that in his lecture , have to double check it but as far as my memory takes me if penecillin is causing rash then it HS 1 but when it causes nephritis its 3 so it can cause both types.

if u r facing problem in the pathogenesis og HSs then go to immunology and check out the hypesensitivity section they have explained it really well.

yea i thought nephritis was 3 also but this guy said 4 so i thought maybe 4 more =/ but i know 4 is in there also because of time delay
