A 68 yo woman has had fatigue for 3 months. She is on digoxin, thiazide, enalapril and naproxen for her CAD and arthritis. She also had amox for her recurrent bronchiti. Her BUN = 60, creatinin = 6. Which of the following caused her conditon ?

A. Amox

B. Digoxin

C.Enalapril

D.Thiazide

E.Naproxen

Giving explain , plz !

It is Naproxin :

Explanation is as follows :

In three conditions Renal circulation is maitained by PGI2 and PGE2 :
  1. Shock
  2. Old age
  3. Congestive Cardiac failure

Consequently in all these three condition NSAIDS are like to compromise these Prostaglandin production leading to renal failure

Those drugs which compromise Renal ciculation i.e they cause Vasoconstriction are likely to produce picture of Pre-renal azotemia
  1. NSAIDS
  2. Costrast agent
  3. Cyclosporin

Just to Add

Indomethacin is the most dangerous in this matter

Aspirin is least dangerous

Rest of NSAIDS including Naproxin and Ibuprofen intermediate in causing this renal vasoconxtriction .

The only NSAIDS which is renal sparing is SUlindac
