Obesity 32 yo lady, nullparity, with infertility, menorrhea, with 2 periods between 1- 6 months and each one lasts longer than 2 weeks. She also has hirsutism. Her left lower pelvis manual exam shows large mass in and US reveals multi-cystic in left overian. What would be best next step in management ?
A. Measurement of morning cortisol
B. Dexamathaxone test
C. Measurement of testosterone and DHEA-S
D. Hysterectomy
E. Exploration laparotomy

A multi-cistic mass in a woman should always be a warning sign of ovarian carcinoma ,
It needs excision

she is young and not having any child ?
Most of the PCOS is secondary so do we need to find out the reason ?

Could you please rewrite the sono of the patient ?
It says multiple cysts or a multicytic mass??
They are completely different!

This definately sounds like PCOS.
Ovarain cancer doesnt usualy presnt at this age of 32.
Plus obesity, nulliparity, menstraul irr, hirsutism etc points to PCOS.
i would go with C.

cos testosterone will be ovarian etiology and DHEA-S is purely adrenal etiology.

if ovary enlarge in post menopausal women then first suspicion would be malignancy but here everything goes with PCOS
