what effect it has on one's application if we select both types of training i.e. prelimanary and categorical ?

They dont cost more. The cost is same if they are part of the same institute.

If you are applying to prelim progam , you usually need to give your PGY2 interest in some programs like Ophthalm., Neuroloy( Advanced) etc. Then u have difficulty explaining why you are applying for IM categorical.
