a pt on examination =diaphoretic n muscle rigidity ,taking some medication for anxiety n paranoia for 5 yrs ,now shows elevated BP n Creatine kinase ,bromocriptine makes her responsive now ............whts the medication she was taking??

r u sure?bcoz thiothixene causes hypotension .......
above symptoms point towards neuroleptic malignant syn. which is due to neuroleptic drugs.
i n this , there is ms rigidity , altered mental status, and autonomic dysfunction.
The diagnosis is confirmed, but not necessarily excluded, by the presence of the following 5 criteria:
Recent treatment with neuroleptics within past 1-4 weeks
Hyperthermia (above 38°C)
Muscular rigidity
At least 5 of the following: Change in mental status
Tachycardia
Hypertension or hypotension
Diaphoresis or sialorrhea
Tremor
Incontinence
Increased creatinine phosphokinase (CPK) or urinary myoglobin
Leukocytosis
Metabolic acidosis
Exclusion of other drug-induced, systemic, or neuropsychiatric illness

Cause by

Halothane group, Succinylcholine, Haldol, Phenothaizines, ** Thiothixene is a traditional antipsychotic Lidocaine, Mepivicaine

Dantolene treats it.

Clozapine inhibits 5HT
Fluoxetine is SSRI
The one that begins with Q -I DO NOT know

yeah it is thiothixene(bcoz it acts on DA receptors)
Clozapine does not exhibit significant antagonism of D2 dopamine receptors in the nigrostriatal tracts so is less likely to cause NMS.

I thought this was serotonin syndrome caused by SSRI like Fluoxetine??
