a 30yr old woman complains of gradual decrease in vision of the right eye over the last week with discomfort on moving the right eye.Ph.E:unable to adduct eyes on lateral gaze,swollen right optic nerve with blurred margins.V/A:20/400 for the right eye,V/A:20/20 for the left eye.DTR r asymmetric,hyperactive.upon flexion of neck she reports feeling electric shocks down her spine.
What's the most likely diagnosis?

It looks like multiple sclerosis.

yep..looks like neuromyelitis optica..devics disease( multiple sclerosis variant)
