Which is least likely seen in Sarcoidosis

A. Kidney stone
B. Granulomatosis hepatitis
C. Myocarditis
D. Bell's palsy
E. ANA

Kidney stones yes because of elevated CA
I will go with D Bell palsi

Kidney stone: not mentioned. Sarcoidosis does increase calcium levels in blood and urine, so does predispose kidney stone. Sarcoidosis is the one most commonly
associated with nephrolithiasis among granulomatous diseases

Liver: granuloma commonly found histologically

Heart: 5-10% (20-30% autopsy) conduction, arrhythmia, heart failure.
* E-med says "Heart failure from cardiomyopathy rarely occurs"

Bell's palsy: MC neurologic finding (5-10%)
* Again E-med says "neurologic manifestation is rare"

ANA: 1/3 +ive

A. Can be seen because of hypercalcemia.
B. MC non-infectious cause.
C. sarcoid may produce it
D. May be seen as a CNS manifestation
E. May be increased

I suppose it's a matter of percentages for each one, i have no idea. I'd go with bell's palsy just because I think CNS involvement is worse and should be uncommon.

A. Kidney stone - Hypercalciuria occurs in only 1-2% of all patients. Only some will develop kidney stones.
B. Granulomatosis hepatitis - Liver invlovement can be seen in upto 60-90% of patients
C. Myocarditis - 5% patients have significant heart involvement
D. Bell's palsy - 5% patients have CNS involvement, 7th nerve invovement being the most common
E. ANA - around 1/3 of patients

So, it seems like kidney stone is the least likely.

my initial guess was Bells but after reading I like stones like stones since stone formation could be ue to :

1) idiopathic hypercalciuria-50%

2)hyperuricosuria(with and without gout)-30%

3)hyperoxaluria-15%

4)low urinary citrate excretion-50%

in so many word stone formation is almost multifactorial eg.--solubility, concentration and urine pH characteristics,- citrate, pyrophosphate and magnesium inhibit stone formation,- protein matrix ,uroepithelium, renal tubular epithelium.. hope thi helps..
