Hi!
I had few detailed questions about shoulder exam. Although they might seem too detailed they might really be important ....

1. In shoulder exam, do we have to ask the patient to take the gown completely off, as shown in the USMLE world videos, or we untie the gown and can ask him/her to lower the gown?

2.If the patient is in severe pain and doesn't want to be examined, after explaining to him/her that it is very important could we still continue with probably painful manouvers such as crossover test,passive range of motion for impingement and muscle force(which is very painful for uch patient) or not.

The reason I am asking is that I had such case when I took CS last time and I was really gentle and explained everything to him, but I failed so I think may be I was too pushy in examination(even though gently)

Isn't it better if we just ask him to show us how he can far he could move his arms (active range of motion) and just test the sensation and reflexes?

3. not really related to shoulder pain but in general do we also have shake hands before leaving the room?

I really appreciate your input as I have the exam in 2 days!!

smiling facei am putting my opinion down,first expose as much as is required, coz you donot gain anything by removing whole gown and also for all that time you will take to perform all the tests, that'll make SP uncomfortable.
also being pushy is last thing to be with a SP, or for that matter any patient, that is interpreted as lacking compassion.
shaking hands at the time of leaving i think is must as it is your last warm gesture that you show the SP you spent last 15minutes with and sort of assures you the support you may get while he/shechecks that check list.lots of good luck
